Position Overview
Join Altrix Group as a Virtual Admission & Discharge Nurse and help support bedside care teams through technology-enabled nursing services. In this remote role, you will collaborate with hospital staff to facilitate admissions, discharges, and patient transfers while ensuring patients receive timely education, care coordination, and discharge support. This position plays a critical role in improving patient experiences, operational efficiency, and care transitions.
Key Responsibilities
- Complete admission and discharge workflows according to facility protocols.
- Conduct admission and discharge assessments using telehealth technology.
- Develop and update nursing care plans appropriate to each patient.
- Collaborate with charge nurses, unit leadership, and interdisciplinary care teams to prioritize workflow.
- Support discharge and transfer processes.
- Assist in ensuring medication information is accurate and complete.
- Help coordinate discharge instructions and follow-up care plans.
- Support multiple patient care units based on admission and discharge volume.
Qualifications
- Graduate of an accredited nursing program.
- Current RN or LPN license in good standing.
- Minimum of two years of clinical experience.
- Current Basic Life Support (BLS) certification.
- Experience using EMR/EHR systems and telehealth platforms.
- Strong communication, critical thinking, organization, and time-management skills.
- Ability to work independently and prioritize effectively.
- Access to a private workspace and reliable internet connection.
- Ability to pass a background check and drug screen.
