
Position Overview
Help patients successfully transition from hospital to home through proactive follow-up, patient education, and timely identification of questions or concerns that may affect recovery.
Key Responsibilities
- Conduct follow-up calls with recently discharged patients.
- Review patient health status and recovery progress.
- Confirm medication understanding and adherence.
- Verify upcoming appointments and follow-up care plans.
- Discuss home-based services and support needs.
- Identify patient concerns, misunderstandings, or potential barriers to recovery.
- Document all patient interactions accurately and thoroughly.
- Escalate concerns according to established protocols when appropriate.
Qualifications
- Fluent English communication skills, both written and verbal.
- Experience with EMR/EHR systems and basic computer applications.
- Strong patient-service mindset and communication skills.
- Critical thinking and independent decision-making abilities.
- Excellent organizational and time-management skills.
- Ability to work independently and prioritize tasks effectively.
- Knowledge of medical terminology preferred.
- Access to a private workspace and a reliable internet connection.
- Ability to pass a background check and drug screen.
