Discharge Care Specialist

Position Overview

Help patients successfully transition from hospital to home through proactive follow-up, patient education, and timely identification of questions or concerns that may affect recovery.  

Key Responsibilities

  • Conduct follow-up calls with recently discharged patients.
  • Review patient health status and recovery progress.
  • Confirm medication understanding and adherence.
  • Verify upcoming appointments and follow-up care plans.
  • Discuss home-based services and support needs.
  • Identify patient concerns, misunderstandings, or potential barriers to recovery.
  • Document all patient interactions accurately and thoroughly.
  • Escalate concerns according to established protocols when appropriate.  

Qualifications

  • Fluent English communication skills, both written and verbal.
  • Experience with EMR/EHR systems and basic computer applications.
  • Strong patient-service mindset and communication skills.
  • Critical thinking and independent decision-making abilities.
  • Excellent organizational and time-management skills.
  • Ability to work independently and prioritize tasks effectively.
  • Knowledge of medical terminology preferred.
  • Access to a private workspace and a reliable internet connection.
  • Ability to pass a background check and drug screen.
