Virtually collaborates with the bedside nursing and healthcare team, providing high-quality patient care services and managing all aspects of admission, discharge, and transfer for patients within their assigned areas.
Your Responsibilities:
- Processes admission and discharge workflows in accordance with facility protocols.
- Utilizes EMR / EHR / telehealth to complete the admission/discharge assessment.
- Initiates and develops nursing care plans, including medication reconciliation, appropriate to the patient.
- Must hold an active RN or LPN license and have experience using remote health documentation systems.
- Supports multiple units based on admission and discharge volume.
- Collaborates with charge RNs and unit leadership to determine priority workflow.
