How we do it
Virtual Admission Nursing:
Monitors emergency department (ED) electronic boards and communicates with the nursing and healthcare teams as patients transition from the ED to the floor.
Connects with the floor nurse to provide documentation assistance, utilizing a tablet or phone to contact the patient and complete the medical health assessment.
Reviews medications with the patient/family/caregiver and contacts the preferred pharmacy or provider, requesting current medication lists be faxed to the hospital.
Follows up with the floor nurse, handing over pertinent patient information.
Virtual Discharge Nursing:
Confirms notification that patient is ready for discharge.
Verifies discharge orders and reviews care coordination plans.
Identifies medications, checking for new or changed medications/dosages, providing verification that they were received and the schedule for medications to be taken upon discharge.
Provides education to patients on discharge diagnosis and confirms care instructions, medication information, and follow-up appointments.
