Virtual Admission & Discharge Nurse

Virtually collaborates with the onsite nursing and healthcare team, providing high-quality patient care services and managing all aspects of admission, discharge, and transfer for patients within their assigned areas.

Your Responsibilities

Processes admission and discharge workflows per facility protocol.
Initiates and develops nursing care plans, including medication reconciliation appropriate to patient.
Collaborates with charge RNs and unit leadership to determine priority workflow.
Moves between assigned units as needed per admission/discharge volumes.
Utilizes EMR/EHR/telehealth to complete the admission/discharge assessment.

Post-Discharge Follow-Up Specialist

As a post-discharge follow-up specialist, you will be crucial in identifying and addressing patients' actions, questions, misunderstandings, and discharge plan information and discrepancies.

Your Responsibilities

In this role, you will proactively manage and meticulously document follow-up calls for patients recently discharged from acute care facilities. Your responsibilities will encompass a thorough review of each patient’s:

Health status
Medication regimen
Upcoming appointments
Home-based services
Plans to address potential problems
