Virtual Admission & Discharge Nurse
Your Responsibilities
Processes admission and discharge workflows per facility protocol.
Initiates and develops nursing care plans, including medication reconciliation appropriate to patient.
Collaborates with charge RNs and unit leadership to determine priority workflow.
Moves between assigned units as needed per admission/discharge volumes.
Utilizes EMR/EHR/telehealth to complete the admission/discharge assessment.
