Virtual Admission and Discharge Nursing

01. What is a Virtual Admission/Discharge Nurse?
Virtual Admission and Discharge Nurses are licensed nurses who work remotely to assist with patient transitions. They manage admission and discharge documentation within the hospital’s EMR, coordinate with bedside teams, and support throughput goals.
02. Does a Virtual Admission and Discharge Nurse replace the bedside nurse?
No. Virtual nurses provide support to bedside nurses by managing documentation and non- direct care tasks. This allows bedside nurses to spend more time with their patients.
03. What are some of the benefits of a Virtual Admission/Discharge Nurse?
Virtual nurses help save time, reduce burnout, and improve discharge efficiency. Hospitals that utilize virtual support often report faster discharge turnaround times, improved compliance with documentation standards, and higher staff satisfaction.

Post-Discharge Follow-Up and Call Navigator

01. What is the Call Navigator?
The Call Navigator is a web-based platform that guides Altrix specialists through structured post-discharge follow-up calls. It includes risk flagging, standardized scripts, escalation protocols, and outcome tracking. It does not require EMR integration.
02. Are your post-discharge specialists licensed nurses?
No. Our post-discharge specialists are trained care coordinators with a healthcare background. They follow structured call protocols using the Call Navigator platform and escalate clinical concerns to appropriate medical personnel.
03. Does the Call Navigator integrate with our EMR?
No. The Call Navigator operates as a standalone platform. It is designed to function independently of your EMR, while allowing for summary reports to be shared with your care team when needed.
04. How is follow-up data tracked or escalated?
Call outcomes are recorded in the Call Navigator. Risks are flagged in real time, and escalation protocols route clinical concerns to the appropriate care team members based on your organization’s workflow.
