As a Post-Discharge Care Specialist, you will proactively engage and document calls with patients recently discharged from acute care facilities. You will follow structured protocols to assess recovery, confirm medications and appointments, and identify potential risks—ensuring patients have the support needed for a safe transition home.
What You'll Do
Conduct structured phone calls with recently discharged patients.
Assess and document each patient's health status, medications, upcoming appointments, and home-based services.
Identify potential concerns and escalate issues using standardized escalation pathways.
Record all patient interactions using our secure Call Navigator platform.
Ensure accuracy and completeness in all documentation.
Follow HIPAA-compliant communication practices.
