As a Virtual Admission and Discharge Nurse, you will work remotely to support bedside nurses and healthcare teams by managing key patient transitions. You will complete documentation and workflows for admissions, discharges, and transfers, contributing to safer, more efficient care.
Process admission and discharge workflows per facility protocol.
Collaborate with charge RNs and unit leadership to determine priority workflow.
Utilize EMR/EHR/telehealth platforms to complete admission and discharge assessments.
Initiate and develop nursing care plans, including medication reconciliation, tailored to each patient's specific needs.
Support multiple units based on admission and discharge volume.
Work with patients, pharmacies, or facilities to ensure medications are properly entered prior to arrival.
Coordinate with the interdisciplinary care team to confirm discharge medications, instructions, and follow-up care.
Document all required information accurately and completely within the EMR.
