Virtual Admission & Discharge Nurse
What You’ll Do
As the Virtual Admission and Discharge Nurse, you will integrate and collaborate with the onsite nursing and healthcare team, producing a cohesive team to address the patient’s needs. This position provides high-quality patient care services and increases satisfaction by facilitating and managing all aspects of admission, discharge, and transfer for patients within their assigned areas.
- Processes admission and discharge workflows per facility protocol.
- Initiates and develops nursing care plans appropriate to the patient.
- Collaborates with charge RNs and unit leadership to determine priority workflow.
- Moves between assigned units as needed per admission/discharge volumes.
- Utilizes EMR/EHR/telehealth to complete the admission/discharge assessment.
- Assists in the discharge/transfer process.
- Works with patients, pharmacies, or facilities to ensure medications are properly entered in the medication section before arrival.
- Works with the interdisciplinary care team to ensure accurate discharge medications, discharge instructions, and follow-up care as volumes allow.
What We’re Looking For
The Virtual Admission and Discharge Nurse must be a registered/licensed nurse with relevant working experience within a hospital.
- Graduate of accredited nursing program.
- 2+ years of clinical experience.
- Current license in good standing to practice as a Registered/Licensed Practical Nurse in the state required.
- Must possess and maintain current Basic Life Support (BLS) certification or obtain certification before the start date.
- Writes, reads, comprehends, and speaks fluent English.
- Proficient in EMR/EHR and basic computer skills.
- Customer/patient-focused.
- Critical thinking skills using independent judgment in making decisions.
- Very good organizational competencies.
- Ability to prioritize.
- Efficient in time management, analytical thinking, and attention to detail.
- Must have access to a completely private workspace with reliable internet.
- Must be able to pass a drug screen and background check.
