Post-Discharge Follow-up Specialist
What You’ll Do
As a Post-Discharge Follow-up Specialist, you will proactively manage and meticulously document patient calls for those recently discharged from acute care facilities. Your responsibilities will encompass a thorough review of each patient’s:
- Health status
- Medication regimen
- Upcoming appointments
- Home-based services
- Plans to address potential problems
What We’re Looking For
We are seeking individuals who possess the following qualifications and qualities:
- Proficiency in reading, writing, comprehending, and speaking fluent English
- Familiarity with EMR/EHR systems and basic computer skills
- Strong customer and patient-focused mindset
- Exceptional critical thinking abilities, utilizing independent judgment in decision-making
- Excellent organizational competencies
- Ability to prioritize tasks and work independently
- Proficient in time management, analytical thinking, and attention to detail
- Access to a completely private workspace with reliable internet
- Ability to pass a drug screen and background check
- Medical terminology knowledge is a plus
