Our nurse scribes are paraprofessionals who specialize in charting physician-patient encounters in real-time, such as during the admission and discharge process and medical examinations. In real time, we document the patient encounter from start to finish, including patient history, physical exams, consults and care plans as well as disposition. How we do it
Virtual Nurse Scribe:
Informed by the onsite facility nurse via iPad or phone that they are to begin the focused/head-to-toe assessment of the patient in their assigned room.
Onsite nurse places the phone at bedside/in pocket and verbalizes the information as she interacts with the patient.
In a routine format, the nurse scribe checks off the information in the medical record and adds any additional information the onsite nurse feels are pertinent.
